Edit Workflow States - Not capturing Changes when clicking "Submit" button

I’m noticing that the “Submit” button for capturing the changes users/roles don’t always work. It refreshes the screen, but leaves the Users/Roles unchanged. Every once-in-a-while, the changes are captured, but there doesn’t seem to be a rhyme or reason to when this occurs…

I haven’t been able to replicate this issue. I tried changing users and roles but all of them saved. Let me know if this keeps happening but since it seems so random it’s hard to pin down.
